Mid-State’s application is online and can be completed in just a few minutes. Before you begin, review our application checklist to make sure you have what you need. If you prefer, a printable application is available.
The Application
Mid-State’s application is for students looking to start in a degree program:
- Apply if you are planning to enter an associate degree, technical diploma, or certificate program. You may also apply if you have not yet chosen a program (undecided). Have questions or want to get help from a recruiter? Give us a call at 888.575.6782.
- Qualifying high school students can skip the application through our Direct Admission process.
If you just want to enroll in a course or two, visit our Take a Class page. If you are a current student who wishes to add or change programs, make an appointment with your academic advisor by calling 888.575.6782.

Application Checklist
Mid-State’s application is quick to complete. You’ll want to set aside a few minutes to fill out the application and have the following:
Social Security Number
You will be asked to enter your Social Security Number in the application*.
Program Choice
You will indicate your program of choice or indicate Undecided in the application. You can change this during the advising phase of the admissions process. Not sure which program to choose? Start on our Career Clusters page to narrow down the options.
