Transcript Request

Requests for official transcripts can be made online through MyMSTC or in person from any of Mid-State's four locations. You may also submit your request in writing via email, fax, or mail. There is a $6 fee for "RUSH" processing of official transcripts. Financial obligations to the college must be met before a transcript will be issued. 

Note: We cannot receive transcript requests by telephone. Also, in accordance with the Family Educational Rights and Privacy Act (FERPA), we do not fax transcripts.

Below are detailed instructions for requesting your transcripts. If you have questions or need more information please contact a college experience specialist at 715.422.5300.

How to Request Transcripts 

  1. Sign into MyMSTC.
  2. Click on the Student Center link in the Self Service menu.
  3. Choose Transcript: Request Official from the dropdown menu and click the double arrows.
Email, fax, or mail

Using one of the following methods, send us a signed letter requesting the release of your transcript. (Include all information listed below under Include with All Requests.)

  • Scan and Email to

  • Mail to the Office of Student Records at our Wisconsin Rapids campus or to another location of your choice:

Include with All Requests

Regardless of how you submit your request, be sure to include the following information to expedite the processing of your transcript:

  • Name and complete mailing address to whom you want the transcript released
  • Complete name (name used at time of enrollment)
  • Student ID or Social Security Number
  • Current mailing address
  • Telephone number
  • Dates of attendance (Note: If enrolled prior to 1983, transcripts are stored on microfilm.)
  • Signature (required)

Official Transcript Processing Fee

There is no fee for standard requests. Transcripts are processed and mailed twice a week. There is a $6.00 fee for on-demand “RUSH” processing (same day service).

GED®/HSED Transcript or Duplicate Certificate/Diploma Requests:

If you need a transcript or a duplicate of a Certificate of General Educational Development® (GED) or High School Equivalency Diploma (HSED) completed in Wisconsin, submit an order form to the Wisconsin Department of Public InstructionThere is a $15 fee for the certificate, diploma, or transcript.
