To apply to the Nursing Assistant program, please submit the following to the MSTC Student Affairs-Admissions Office. |

Include a $30 non-refundable application fee payable to MSTC. |

3. |
Complete an Accuplacer entrance examination. (Other test scores may be acceptable alternatives.) |

Proof of identification (Submit a photocopy of one of the following: birth certificate, driver's license or passport.) |

5. |
Verification of Social Security Number (Submit a photocopy of one of the following: Social Security card, check stub from Internal Revenue Service form.) |

6. |
Completed Background Information Disclosure (BID) Form. |

Wisconsin Caregiver Law (1997 WISCONSIN ACT 27) requires all students with a positive Background Information Disclosure form to have a Caregiver Background Check (nonrefundable $10.00 fee - only submit the $10 fee if you have criminal convictions). Based upon results of the criminal background check, a student may be denied access to clinical agencies and thus would not be able to complete the program or be employed in health care. Also, clinical agencies will be notified of all students with pending charges and convictions and agencies can decline student admission to that agency.
