To apply to the Paramedic Technician program, please submit the following to the MSTC Student Affairs-Admissions Office: |

Include a $30 non-refundable application fee payable to MSTC. |
||||||
3. |
Complete an Accuplacer entrance examination. (Other test scores may be acceptable alternatives.) |

4. |
High School Transcript or HSED/GED test scores. To be eligible for financial aid, EMT-Paramedic applicants are required to submit their high school transcripts or HSED test scores. |

5. |
Must be at least 18 years of age. |

6. |
Completed Background Information Disclosure (BID) Form. |

Wisconsin Caregiver Law (1997 WISCONSIN ACT 27) requires all students have a Caregiver Background Check. (nonrefundable $10.00 fee). Based upon results of the criminal background check, a student may be denied access to clinical agencies and thus would not be able to complete the program or be employed in healthcare. Also, clinical agencies will be notified of all students with pending charges and convictions and agencies can decline student admission to that agency. Military Discharge Form (DD214) required if ever discharged from the military. |

Proof of current State of Wisconsin EMT Licensure of National Registry of EMTs certification. Students are required to hold a current State of Wisconsin EMT license prior to beginning clinical experience. |
